Veterans Services Licensed Clinician

Job Description

Forge Health is seeking a Veterans Services Licensed Clinician to join a collaborative team between Forge Health and the Tunnel to Towers Foundation. This role is based in North Charleston, SC, with an anticipated start date in Fall 2025. The clinician will provide behavioral health services to the Tunnel to Towers Foundation's Charleston Veterans Village, offering permanent affordable housing and comprehensive supportive services to veterans who were previously struggling with homelessness. This is an in-person opportunity for a clinician to work in an outpatient setting, providing individual services, identifying potential groups, conducting intakes, and collaborating with the T2T care team.Role involves:

• Maintaining a caseload of complex Behavioral Health cases.
• Participating in multidisciplinary team meetings and case management.
• Completing initial assessments and determining appropriate levels of care.
• Conducting workshops, trainings, and providing educational resources.
• Preparing and maintaining treatment plans.
• Coordinating with discharge planners.
• Documenting all counseling services in clinical records.
• Providing crisis intervention as needed.

Requirements:

• Valid, unrestricted license in South Carolina as a LISW-CP, LPC, or LMFT.
• Masters or Doctorate Degree in Social Work or a similar field.
• Demonstrated knowledge working with multidisciplinary teams.
