Geriatric Specialty Care of Reno is seeking a Hybrid Primary Care Coordinator (Medical Assistant) to serve as the central point of contact for their primary care geriatric care team. This role involves managing 500-600 patients alongside nurses and Primary Care Providers, coordinating patient care, maintaining relationships with senior living facilities, and ensuring excellent healthcare delivery through effective communication and documentation. The position offers a flexible hybrid arrangement, requiring 1-2 days per week in the office, with additional on-site coverage as needed for clinical operations.
The role involves:
- Serving as a primary contact for patients, families, and providers.
- Scheduling and coordinating medical appointments.
- Managing patient documentation and EMR updates (Epic experience a plus).
- Processing urgent calls and tasks.
- Handling communications via phone, email, text, fax, portal, etc.
- Coordinating with community partners and specialty providers.
- Facilitating new patient onboarding.
Requirements:
- High School Diploma (some college preferred).
- Basic understanding of Primary Care Operations.
- Medical Assistant Certification preferred.
- Reliable internet and HIPAA-compliant workspace.
- Comfort with healthcare technology platforms.
- Ability to thrive in a fast-paced, changing environment.
- Must not be on any exclusion or debarment from participation in Federal Health Care Programs.
