Community Guide - Behavioral Health at Reema Health

Reema Health is seeking a Community Guide to work with individuals living with serious mental illness and substance use disorder. The Community Guide will work with the multidisciplinary care team to plan and coordinate services for members. This role requires significant time in the field, directly interacting with members.

The role involves:

• Engaging and working alongside clients from diverse ethnic backgrounds.
• Assessing member needs and support systems.
• Establishing trusting relationships with members.
• Coordinating referrals and support appointment scheduling.
• Handling a caseload of 60 - 100 members.
• Visiting members in person as appropriate.
• Utilizing text, phone, email and face-to-face interactions.
• Connecting members to community and health resources.
• Completing accurate and thorough documentation.

Requirements:

• Bachelor’s degree in Social Work, Human Services, Psychology, Health Sciences or related field.
• 3+ years of direct experience with the population or community to be served.
• Experience with crisis planning, case management, boundaries, and personal safety.
• Proficiency in critical thinking, time management, and strategizing.
• Strong relationship-building skills.
• Proficiency working with technology.
• Valid Driver's License, Vehicle Insurance, and a satisfactory driving record.
