Obesity Medicine Nurse Practitioner

Job Description

Form Health is seeking an experienced Obesity Medicine Nurse Practitioner to join their virtual obesity medicine clinic. The Nurse Practitioner will deliver multi-disciplinary evidence-based obesity treatment through telemedicine, collaborating with physicians and registered dietitians to provide exceptional team-based care. This is a full-time, remote position.

Responsibilities include:

• Providing longitudinal telehealth care for patients with obesity.
• Collaborating with physicians and registered dietitians to deliver team-based care.

The ideal candidate will have:

• A Nurse Practitioner degree with experience in Obesity Medicine.
• Licensure to practice and prescribe in California.
• 2+ years of experience in outpatient or obesity medicine, endocrinology, or bariatrics.
• Excellent interpersonal skills.
• Commitment to high-quality medical care and an evidence-based approach.
• Experience conducting telemedicine visits.
