Obesity Medicine Nurse Practitioner

Form Health, a virtual obesity medicine clinic, is seeking an experienced Obesity Medicine Nurse Practitioner to provide longitudinal telehealth care to patients. The Nurse Practitioner will collaborate with physicians and registered dietitians to deliver team-based care. This is a full-time, remote position.

Responsibilities:

• Provide longitudinal telehealth care for patients with obesity.
• Collaborate with physicians and registered dietitians to deliver team-based care.
• Provide evidence-based lifestyle counseling and pharmacotherapy.

Requirements:

• Nurse Practitioner degree with experience in Obesity Medicine.
• Licensed to practice and prescribe in at least one of the following states: MA, NY, TX, NJ, and/or IL.
• 2+ years of experience in outpatient or obesity medicine, endocrinology, or bariatrics, and/or 2 + years of obesity medicine experience in a primary care/internal medicine setting.
• Excellent interpersonal skills.
• Commitment to high-quality medical care and an evidence-based approach.
• Prior telemedicine experience.
• Bilingual in Spanish preferred.
