Form Health is seeking an experienced Obesity Medicine Nurse Practitioner to provide longitudinal telehealth care to patients. The Nurse Practitioner will collaborate with physicians and registered dietitians to deliver team-based care. Form Health is a virtual obesity medicine clinic delivering multi-disciplinary evidence-based obesity treatment through telemedicine. The company is committed to providing high-quality expert care and leveraging technology to enhance the patient experience.

Responsibilities:

• Provide longitudinal telehealth care for patients with obesity.
• Collaborate with physicians and registered dietitians to deliver team-based care.
• Provide evidence-based lifestyle counseling and pharmacotherapy.

Requirements:

• Nurse Practitioner degree with experience in Obesity Medicine.
• Licensed to practice and prescribe in California.
• 2+ years of experience in outpatient or obesity medicine, endocrinology, or bariatrics.
• Excellent interpersonal skills.
• Commitment to high-quality medical care and evidence-based approach.
• Experience conducting telemedicine visits.
• Bilingual (Spanish) preferred.
