Obesity Medicine Nurse Practitioner

Job Description

Form Health is seeking an experienced Obesity Medicine Nurse Practitioner to deliver longitudinal telehealth care to patients with obesity. The Nurse Practitioner will collaborate with physicians and registered dietitians to provide team-based care. Form Health is a virtual obesity medicine clinic that delivers multi-disciplinary evidence-based obesity treatment through telemedicine.

• Providing longitudinal telehealth care for patients with obesity
• Collaborating with physicians and registered dietitians
• Delivering team-based care

• Nurse Practitioner degree with experience in Obesity Medicine
• Licensed to practice and prescribe in at least one of the following states: MA, NY, TX, NJ, and/or IL
• 2+ years of experience in outpatient or obesity medicine, endocrinology, or bariatrics, and/or 2 + years of obesity medicine experience in a primary care/internal medicine setting
• Excellent interpersonal skills
• Commitment to high-quality medical care and evidence-based approach
• Prior telemedicine experience
