Form Health, a virtual obesity medicine clinic, is seeking an Obesity Medicine Physician to provide telehealth care and collaborate with a multidisciplinary team. The physician will deliver evidence-based obesity treatment through telemedicine, working closely with advanced practice professionals and registered dietitians. This is a remote position with flexible schedules, requiring a commitment to a full-time schedule across a 5-day work week.

Role Involves:
• Providing longitudinal telehealth obesity medicine and medical weight management care.
• Collaborating with advanced practice professionals and registered dietitians.
• Seeing patients across a 5-day work week.

Requirements:
• MD or DO degree.
• Board Certification in Obesity Medicine (ABOM).
• 2+ years of obesity medicine/medical weight management experience.
• Active medical licenses and credentialing in Florida, New York (NY), Massachusetts (MA), Illinois (IL), and/or Texas (TX). Multi-state licensure is strongly preferred.
• Excellent interpersonal skills.
• Commitment to high-quality medical care and an evidence-based approach.
• Prior telemedicine experience.
• Experience working collaboratively with advanced practice professionals and registered dietitians.
• Bilingual (Spanish) fluency preferred.
