Form Health, a virtual obesity medicine clinic, is seeking an Obesity Medicine Physician to deliver multi-disciplinary, evidence-based obesity treatment through telemedicine. The physician will provide longitudinal telehealth obesity medicine and medical weight management care, collaborating with advanced practice professionals and registered dietitians. Form Health is dedicated to providing high-quality expert care and leveraging technology to enhance the patient experience.

• Providing longitudinal telehealth obesity medicine and medical weight management care
• Collaborating with advanced practice professionals and registered dietitians
• Seeing patients across a 5-day work week

• MD or DO degree
• Board Certification in Obesity Medicine (ABOM)
• 2+ years of obesity medicine/medical weight management experience
• Licensed and credentialed in California (CA)
• Excellent interpersonal skills
• Commitment to high-quality medical care
• Experience with telemedicine
• Experience working collaboratively with advanced practice professionals and registered dietitians
