Obesity Medicine Physician - Remote

Form Health, a virtual obesity medicine clinic, is seeking an Obesity Medicine Physician to deliver multi-disciplinary, evidence-based obesity treatment through telemedicine. This role involves providing longitudinal care, collaborating with advanced practice professionals and registered dietitians, and committing to a full-time schedule. The physician will work remotely, seeing patients across a 5-day work week.Form Health is dedicated to providing high-quality expert care and leveraging technology to enhance the patient experience. • Provide longitudinal, telehealth obesity medicine and medical weight management care.
• Collaborate with advanced practice professionals and registered dietitians for team-based care.
• Conduct telemedicine visits.

Requirements:

• MD or DO degree.
• Board Certification in Obesity Medicine (ABOM).
• Licensed to practice and prescribe legally in California (multiple state licenses a plus).
• Credentialed/enrolled with insurance payers in California.
• Excellent interpersonal skills.
• Commitment to high-quality medical care and evidence-based approach.
• Prior telemedicine experience.
• Experience working collaboratively in a multi-disciplinary model.
• Bilingual in Spanish is highly preferred.
