Obesity Medicine Physician - Remote

Form Health is seeking an Obesity Medicine Physician to deliver telehealth obesity medicine and medical weight management care. The physician will collaborate with advanced practice professionals and registered dietitians to provide team-based care. This is a remote position with flexible schedules, requiring a commitment to a full-time schedule across a 5-day work week.

Role involves:
• Providing longitudinal, telehealth obesity medicine and medical weight management care
• Collaborating with advanced practice professionals and registered dietitians
• Seeing patients across a 5-day work week

Requirements:
• MD or DO degree
• Board Certification in Obesity Medicine (ABOM)
• License to practice and prescribe legally in California (multiple state licenses a plus)
• Credentialed/enrolled with insurance payers in California
• Excellent interpersonal skills
• Commitment to high-quality medical care and evidence-based approach
• Prior telemedicine experience
• Experience working collaboratively in a multi-disciplinary model
• Bilingual (Spanish) preferred
