Form Health, a virtual obesity medicine clinic, is seeking an experienced Obesity Medicine Nurse Practitioner to provide longitudinal telehealth care. The Nurse Practitioner will collaborate with physicians and registered dietitians to deliver team-based care. This is a full-time, remote position.Role involves:

• Providing longitudinal telehealth care to patients with obesity.
• Collaborating with physicians and registered dietitians.
• Delivering team-based care.

Requirements:

• Nurse Practitioner degree with experience in Obesity Medicine.
• Licensed to practice and prescribe in North Carolina (preferred) or other states.
• 2+ years of experience in outpatient or obesity medicine, endocrinology, or bariatrics.
• Excellent interpersonal skills.
• Commitment to high-quality medical care.
• Experience conducting telemedicine visits.
• Bilingual in Spanish preferred.
