Obesity Medicine Physician - Remote

Form Health, a virtual obesity medicine clinic, is seeking an Obesity Medicine Physician to provide longitudinal telehealth obesity medicine and medical weight management care. The physician will collaborate with advanced practice professionals and registered dietitians to deliver exceptional team-based care. This is a remote position with flexible schedules, requiring a full-time commitment across a 5-day work week. Form Health is dedicated to providing high-quality expert care and leveraging technology to enhance the patient experience.Role involves:

• Providing longitudinal telehealth obesity medicine and medical weight management care.
• Collaborating with advanced practice professionals and registered dietitians.
• Seeing patients across a 5-day work week.

Requirements:

• MD or DO degree.
• Board Certification in Obesity Medicine (ABOM).
• 2+ years of obesity medicine/medical weight management experience.
• Licensed to practice and prescribe legally in Tennessee (multiple state licenses a plus!).
• Excellent interpersonal skills for patient and team interactions.
• Commitment to the highest quality of medical care and an evidence-based approach to obesity management.
• Prior experience conducting telemedicine visits and a great "web-side" manner.
• Prior experience working collaboratively with advanced practice professionals and registered dietitians.
• Bilingual candidates with medical fluency in Spanish are highly preferred.
