Obesity Medicine Physician - Remote

Form Health, a virtual obesity medicine clinic, is seeking an Obesity Medicine Physician to provide longitudinal telehealth obesity medicine and medical weight management care. The physician will collaborate with advanced practice professionals and registered dietitians to deliver exceptional team-based care. This is a remote position with flexible schedules, requiring a full-time commitment across a 5-day work week.Role involves:

• Providing longitudinal telehealth obesity medicine and medical weight management care.
• Collaborating with advanced practice professionals and registered dietitians.
• Seeing patients across a 5-day work week.

Requirements:

• MD or DO degree.
• Board Certification in Obesity Medicine (ABOM).
• 2+ years of obesity medicine/medical weight management experience.
• Licensure to practice and prescribe legally in North Carolina (preferred), and credentialed with insurance payers in one or more of the following states: IL, TX, MA, CA, KS, NJ, WA and/or NY (strong preference for multiple state licenses!).
• Excellent interpersonal skills.
• Commitment to high-quality medical care and an evidence-based approach.
• Prior telemedicine experience.
• Experience working collaboratively with advanced practice professionals and registered dietitians.
• Bilingual candidates with medical fluency in Spanish are highly preferred.
