Job Description
Interwell Health is seeking a dedicated RN Care Manager to provide personalized support to patients with chronic kidney disease. This fully remote role involves collaborating with healthcare providers and dialysis teams to create individualized care plans, educate patients, and coordinate transitions to dialysis. The RN Care Manager will utilize telephonic communication and motivational interviewing techniques to engage patients in their care and reduce hospitalization risks.
What this role involves: - Collaborating with clinical and non-clinical staff to develop individualized interventions.
- Communicating and coordinating with the patient's healthcare team.
- Utilizing motivational interviewing techniques.
- Collaborating with providers to ensure a successful transition to dialysis.
- Partnering with providers and dialysis clinics to mitigate hospitalization and mortality risk.
- Participating in quality improvement activities and project-based work.
- Evaluating program processes and making recommendations for improvement.
Requirements: - Registered Nurse (RN) with active unrestricted licensure.
- A clear and active Nurse Licensure Compact (NLC) is required
- Associate Degree in Nursing (ADN) required; Bachelor of Science in Nursing (BSN) preferred.
- 2 to 5+ years of Nephrology Nursing and/or Case Management experience.
- Knowledge of teaching-learning process and adult education methods.
- Understanding of renal and diabetes disease process.
- Familiarity with telehealth platforms and electronic health records (EHRs).
- Knowledge of healthcare regulations, including HIPAA.
- Excellent communication and interpersonal skills.
- Empathetic and culturally competent approach.
- Ability to prioritize tasks and adapt to changing situations.
