RN Care Manager (Bilingual)

Job Description

Interwell Health is seeking a Bilingual RN Care Manager to join their team. In this remote role, the Care Manager will empower members to manage their health, providing personalized support to slow disease progression and minimize costly interventions. The Care Manager will collaborate with healthcare providers, dialysis teams, and other professionals to create individualized care plans, educate patients, and coordinate transitions to dialysis. This role primarily involves telephonic communication, using motivational interviewing to engage patients and reduce hospitalization risks.Role involves:

• Collaborating with clinical and non-clinical staff to develop individualized interventions.
• Communicating and coordinating with the patient's healthcare team.
• Utilizing motivational interviewing techniques.
• Collaborating with providers and members for optimal transitions to dialysis.
• Partnering with providers and dialysis clinics to mitigate hospitalization and mortality risk.
• Participating in quality improvement activities and education initiatives.
• Evaluating program processes and making recommendations for improvement.

Requirements:

• Registered Nurse (RN) with active unrestricted licensure.
• Associate Degree in Nursing (ADN) required; Bachelor of Science in Nursing (BSN) preferred.
• 2 to 5+ years of Nephrology Nursing and/or Case Management experience.
• Bilingual proficiency (Spanish) required.
• Knowledge of teaching-learning process and adult education methods.
• Understanding of renal and diabetes disease process and management practices.
• Familiarity with telehealth platforms and electronic health records (EHRs).
• Knowledge of healthcare regulations, including HIPAA.
• Excellent communication and interpersonal skills.
• Empathetic and culturally competent.
• Ability to prioritize tasks and adapt to changing situations.
