Herself Health is seeking a Primary Care Nurse Practitioner or Physician Assistant to join their team in Minneapolis, MN. This role involves evaluating patients' acute and chronic conditions, managing medications, addressing mental health and social-emotional needs, and providing patient education. The ideal candidate will partner with the care team to ensure high-quality patient care in accordance with the company's values.

Requirements:
- Licensed Nurse Practitioner (FNP-C or AGNP) or Physician Assistant (PA-C)
- 3-5 years of experience managing complex diseases
- Understanding of value-based care
- Experience with geriatric medicine, particularly women
- Experience with poly-pharmacy management
- Strong problem-solving and critical thinking skills
- COVID-19 vaccination
