Herself Health is seeking a Primary Care Nurse Practitioner/Physician Assistant to join their team in Roseville, MN. Herself Health is dedicated to providing patient-centric primary care for women 65+, focusing on specialized needs such as post-menopausal care, bone density, and mental well-being. The role involves evaluating patients, managing medications, and partnering with the care team to deliver high-quality care.
  • Evaluate patients' acute and chronic conditions.
  • Manage multiple medications and mental health needs.
  • Educate patients in a thorough and empathetic manner.
  • Partner with the care team to provide excellent patient care.
  • Licensed Nurse Practitioner (FNP-C or AGNP) or Physician Assistant (PA-C).
  • 3-5 years of experience managing complex diseases.
  • Understanding of value-based care.
  • Experience with geriatric medicine, particularly women.
  • Valid Minnesota Driver’s License.
  • Competitive salary ($120k-$180k+).
