Job Description
Central California Alliance for Health is seeking a Medicare Care Management Manager (RN) to join their Care Management Department. This role involves planning, organizing, managing, and leading the Care Management Medicare team, acting as a subject matter expert, and providing guidance related to care management activities. The Manager will provide management oversight related to the Care Management Medicare Unit functions and departmental operations, perform program effectiveness evaluation, and manage, supervise, mentor, and train assigned staff. They will also collaborate with a variety of Alliance staff on special projects.

Requirements for this position include:
- Current and unrestricted license as a Registered Nurse issued by the state of California
- Bachelor’s degree in Nursing
- Six years of experience in a health plan or patient care setting, which included a minimum of two years of experience supervising and training a multi-disciplinary health care team, one year of case management experience, and some experience in a care management capacity (a Master’s degree may substitute for two years of the required experience); or an equivalent combination of education and experience may be qualifying
- Knowledge of clinical nursing, case management, Medi-Cal, Medicare D-SNP, and managed care principles
- Ability to analyze regulations, train staff, provide leadership, and present data effectively
