Teen and Young Adult Case Manager

Job Description

• Meeting basic needs for food, clothing, shelter, and medical care.
• Assisting with applications for income, medical, and housing benefits.
• Arranging transportation for clients to access services and resources.
• Providing psychosocial services to improve clients’ abilities to function in social roles.
• Guiding clients in daily living skills, social skills, and leisure activities.
• Assisting clients in finding employment opportunities and supportive housing.
• Offering support and education to families, friends, and community agencies.
• Identifying and working with natural support systems.

• Bachelor's or Master's degree in psychology, social work, or related field.
• One year of experience working with adolescents and/or young adults.
• Valid Colorado Driver's license and clean driving record.
• Knowledge of psychotherapy methods and community resources.
• Ability to assess crisis situations and intervene appropriately.
• Familiarity with the DSM V and diagnostic techniques.
• Understanding of HIPAA principles and confidentiality.
• Effective communication skills and ability to work flexible hours.
