Job Description
Author Health is seeking a Patient Resource Specialist to join their Care Management Team. This role focuses on supporting patients with serious mental illness and substance use disorders by connecting them with vital resources and removing barriers to care. The Patient Resource Specialist will collaborate with a multidisciplinary team to improve access to care and overall health outcomes for patients and their families. This is a hybrid role, primarily remote, but may require travel to patients’ homes as needed.
Role involves:
- Acting as a liaison between patients and community resources.
- Working with care managers and healthcare providers to address patients' needs.
- Assisting patients with applying for Medicaid and other public assistance programs.
- Maintaining accurate records of patient interactions.
- Educating patients about available resources.
- Monitoring patients' progress in securing needed resources.
- Collaborating with multidisciplinary teams.
Requirements:
- Passion for helping patients.
- Compassionate and empathetic attitude.
- Proven ability with working with patients with complex needs.
- High School Diploma or equivalent.
- At least 2 years of experience in case management, social work, customer service, or a related field.
- Strong understanding of social determinants of health.
- Excellent interpersonal and organizational skills.
- Proficient with technology, including Google tools.
