Job Description
West Cancer Center is seeking a Care Support Coordinator to guide patients through the healthcare system, helping them overcome barriers to care and minimize fragmentation. The Care Support Coordinator will work closely with patients, families, and supportive care teams within West Cancer Center, as well as community resources, to ensure seamless coordination of care.

The role involves:
  • Guiding high-risk patients to ensure timely and coordinated care.
  • Identifying barriers to care and connecting patients with appropriate resources.
  • Following up with patients regarding referrals and assistance programs.
  • Maintaining a comprehensive list of patient resources.
  • Assisting patients with navigating insurance and financial assistance programs.
  • Addressing Social Determinants of Health (SDOH).
  • Coordinating transportation services for patients.
  • Providing education on patient portal access.
  • Maintaining accurate documentation in the EMR system.

Requirements:
  • Bachelor’s degree in social work or related field (preferred).
  • At least two years of healthcare experience working directly with patients.
  • Strong organizational and follow-up skills.
  • Excellent verbal and written communication skills.
  • Proficiency in EMR systems, word processing, and email applications.
  • Strong problem-solving skills and the ability to prioritize tasks.
  • Ability to work independently and collaboratively.
  • Ability to assess and address Social Determinants of Health (SDOH).
