Outreach Liaison - Jacksonville, Florida

Job Description

Author Health is seeking an Outreach Liaison to connect patients with mental health and social services. The Outreach Liaison will meet with patients in the community, at their homes, primary care physician offices, and local hospitals. They will build relationships, help patients schedule appointments, and ensure a seamless transition to the clinical team.

• Conduct in-person outreach to educate potential patients and caregivers.
• Independently organize and prioritize workload.
• Utilize Author Health’s CRM and electronic medical record.
• Work collaboratively with internal outreach and care teams.
• Aid in capturing data and providing feedback on outreach strategies.
• Participate in virtual meetings.

• 3+ years of experience in customer service/sales or healthcare outreach.
• Strong ability to initiate conversations and build connections.
• Experience in healthcare, social services, or mental health services.
• Proficiency in critical thinking, time management, and multitasking.
• Able to work independently and meet monthly outreach goals.
• Enthusiasm for working in a fast-paced startup environment.
• Access to a car and ability to drive within a 100-mile radius.
• Strong tech proficiency and comfort using multiple software systems.
• Basic proficiency in Google Suite and video conferencing platforms.
• Adherence to HIPAA regulations and confidentiality.
