Job Description
Arizona Liver Health (ALH) is seeking a VEN Centers Outreach RN Lead to join their team. This role is pivotal in combatting viral hepatitis in Arizona by providing direct patient outreach and removing obstacles to treatment. The VEN Centers Lead Outreach RN will work to ensure patients receive the necessary care.
The role involves significant travel (50-75%) to pop-up sites and requires a passion for patient advocacy.
Role Involves:
- Coordinating with health departments and community groups for outreach.
- Conducting outreach to high-risk communities.
- Providing education and risk reduction plans.
- Completing patient testing for Viral Hepatitis and STIs.
- Conducting psychosocial assessments.
- Supporting patients in treatment.
- Identifying and re-engaging patients who have fallen out of care.
- Maintaining documentation and patient tracking.
- Arranging group education sessions.
Requirements:
- Registered Nurse (RN) license in good standing.
- Bachelor's degree in a relevant field.
- Proficiency in Microsoft applications and EMR systems (eCW).
- Excellent communication skills.
- Ability to work independently in a remote environment.
- Knowledge of public health practices and infectious diseases.
- Phlebotomy experience.
- Bilingual (Spanish-speaking) preferred.
