Job Description
Interwell Health is seeking a Program Manager to join its Provider Operations Team. In this role, the Program Manager will serve as a liaison between the company's provider network and nephrology practices, facilitating a smooth transition to Value-Based Care. The ideal candidate will possess a diligent work ethic, excellent communication skills, and the ability to work independently and collaboratively with external partners and internal, cross-functional teams.
Responsibilities: - Bring deep understanding of the objectives and complexities of intricate Value-Based Care Programs and/or agreements.
- Serve as primary contact and subject matter expert for IWH Network practices.
- Assist nephrology partners in achieving success within value-based care by uncovering valuable insights within data and identifying opportunities for improvement.
- Execute company initiatives, ensuring the timely completion of deliverables.
- Maintain accurate documentation of provider interactions/visits.
- Deliver timely training and/or onboarding to Network Providers.
- Verify that information presented to providers is both accurate and timely.
Requirements: - Bachelor’s degree required; Advanced Degree desirable.
- Proven experience working with physicians and practices, nephrology-specific experience a plus.
- Superior attention to detail and exceptional follow-through.
- Independent and proactive approach, showcasing dedication to continuous learning.
- Excellent communication and interpersonal skills to effectively collaborate with providers, stakeholders, and internal teams.
- Must be willing to travel up to 50% of the time to practices.
