Job Description
Arizona Liver Health (ALH) is seeking a VEN Centers Outreach Specialist/Phlebotomist to join their team. This role is crucial in combating viral hepatitis in Arizona by providing patient outreach and advocacy, ensuring patients receive the necessary treatments. The Outreach Specialist will work remotely with significant travel to pop-up sites, engaging with local health departments and community groups to coordinate services.Responsibilities:
  • Research and coordinate with local health departments and community groups.
  • Conduct outreach to high-risk communities for viral hepatitis.
  • Provide individualized education and risk reduction plans.
  • Complete patient testing for Viral Hepatitis and other STIs.
  • Conduct psychosocial assessments to identify non-medical needs.
  • Provide support to HCV/STI positive persons.
  • Identify and re-engage patients who have fallen out of care.
  • Maintain accurate record-keeping and monthly reports.
Requirements:
  • Bachelor's degree in public health, community health, nursing, social work, or related field.
  • Experience in infectious disease, substance treatment, or human services preferred.
  • Proficiency in Microsoft applications and EMR systems (eCW).
  • Excellent communication skills and ability to work independently.
  • Phlebotomy experience required.
  • Bilingual (Spanish-speaking) highly preferred.
