Job Description
Lantern is seeking a Clinical Care Specialist - Infusions to enhance the patient experience for members receiving infusion care. This role involves shifting patients from hospital settings to more convenient and safer environments, such as their homes or ambulatory infusion centers, while providing engaged clinical support. The Clinical Care Specialist will directly engage with patients to support their selection of top-quality infusion benefits, driving improved patient experience and stronger utilization of Lantern's services. This position requires a professional RN or PA with infusion experience to join a clinical team that offers individualized assistance to patients throughout their infusion journey.
Responsibilities:
- Conducting outreach to members that are candidates for infusion therapy through Lantern
- Educating the patient on the program and the quality of AIC and home infusion care
- Articulating the quality of the specific providers recommended to the patient
- Answering questions/addressing concerns related to our providers
- Single point of contact for the clinical related questions based on patient needs
Requirements:
- Minimum of 5 years of progressive practice experience with a particular focus in infusions
- Bachelor of Nursing Degree or Physician Assistant Degree preferred
- Experience with infusion therapy required
- Knowledge in navigation health programs and demonstrated experience communicating with patients in this regard
