Job Description

Forge Health is seeking a Licensed Mental Health Therapist to join their growing team in Paramus, NJ. The therapist will provide best-in-class mental health services via telehealth and in-person, treating addiction and mental health conditions. • Facilitates outpatient and intensive outpatient programs for mental health and substance use disorders.
• Conducts individual and family therapy, as well as case management.
• Assesses clients for co-occurring and mental health needs using DSM criteria.
• Completes initial assessments and determines appropriate level of care.
• Develops and maintains treatment plans in conjunction with the care team.
• Coordinates with discharge planners regarding continuing care.
• Documents all counseling services in clinical records.
• Participates in multidisciplinary team meetings and case management.
• Conducts individual and group counseling.
• Communicates with family members, attorneys, and other outside resources as needed.
• Provides crisis intervention and other functions as needed.

Requirements:

• Valid, unrestricted license in New Jersey as a LCSW or similar credentials including LSW, LAC, LPC or LMFT.
• Master's Degree in Social Work or a similar field.
• Minimum of two evening shifts per week (may be in person).
