Job Description

Forge Health, a mission-driven outpatient mental health and substance use provider, is seeking a Licensed Mental Health Therapist to join their growing, collaborative team. The therapist will provide best-in-class mental health services via telehealth and in-person at the Rego Park, Queens, NY office, reporting to the Clinic Director. • Facilitating outpatient and intensive outpatient programs for mental health and substance use disorders.
• Conducting individual and family therapy.
• Performing case management for individual clients.
• Assessing clients for co-occurring and mental health needs using DSM criteria.
• Completing initial assessments and determining appropriate levels of care.
• Maintaining treatment plans and documenting counseling services.
• Participating in multidisciplinary team meetings and providing crisis intervention.

Requirements:

• Valid, unrestricted license in New York as a LCSW or similar (LMSW, LMHC, or LMFT).
• Master's Degree in Social Work or a related field.
