Job Description
Forge Health is seeking a passionate and dedicated Licensed Mental Health Therapist to join their growing team. This part-time position is located in Rego Park, Queens, NY, and reports to the Clinic Director. Forge Health is a mission-driven outpatient mental health and substance use provider committed to delivering high-quality, affordable, and effective care. The therapist will provide best-in-class mental health services via telehealth and in-person, focusing on addiction and mental health conditions.
The role involves:
- Facilitating outpatient and intensive outpatient programs for mental health and substance use disorders.
- Conducting individual and family therapy.
- Providing case management services.
- Assessing clients for co-occurring and mental health needs using DSM criteria.
- Completing initial assessments and determining appropriate levels of care.
- Developing and maintaining treatment plans.
- Coordinating with discharge planners.
- Documenting all counseling services.
- Participating in multidisciplinary team meetings.
- Conducting individual and group counseling.
- Communicating with external resources as needed.
- Providing crisis intervention.
Requirements:
- Valid, unrestricted license in New York as a LCSW, LMSW, LMHC, or LMFT.
- Master's Degree in Social Work or a related field.
