Obesity Medicine Physician - Remote

Form Health is seeking an Obesity Medicine Physician to provide telehealth obesity medicine and medical weight management care. The physician will collaborate with advanced practice professionals and registered dietitians to deliver exceptional team-based care. This is a remote position with flexible schedules, requiring a full-time commitment across a 5-day work week.

Role Involves:
• Providing longitudinal, telehealth obesity medicine and medical weight management care.
• Collaborating with advanced practice professionals and registered dietitians.
• Seeing patients across a 5-day work week.

Requirements:
• MD or DO degree.
• Board Certification in Obesity Medicine (ABOM).
• License to practice and prescribe legally, and are credentialed/enrolled with insurance payers, in any of the following states: NY, MA, TX, IL (multiple state licenses a plus!).
• Excellent interpersonal skills.
• Commitment to high-quality medical care and an evidence-based approach to obesity management.
• Prior telemedicine experience.
• Experience working collaboratively in a multi-disciplinary model.
• Bilingual candidates with medical fluency in Spanish are highly preferred.
