SonderMind is seeking a Care Coordinator to be the primary point of contact for clients seeking mental healthcare. This role involves providing support to clients, working with external partners and healthcare systems, and utilizing technology to connect clients with appropriate therapists. The Care Coordinator will play a crucial role in ensuring clients receive the necessary support throughout their care journey.Responsibilities:
  • Provide primary support for clients seeking mental and behavioral health wellness.
  • Utilize technology to connect clients with therapists and maintain support.
  • Assist with identifying the correct provider for care using a consultative approach.
  • Manage inbound and outbound communications, routing messages as needed.
  • Interact with external partners and healthcare systems to ensure access to care.
  • Conduct outreach to referred clients and provide personalized onboarding services.
  • Assist with care coordination tasks such as appointment scheduling.
Requirements:
