Job Description
Herself Health is seeking a Care Coordinator to join their team. This role involves creating care plans for patients, supporting chronic condition management, and addressing medication adherence. The Care Coordinator will work closely with providers and patients, document encounters in the EMR system, and coordinate care with other providers.
Role involves:
- Providing telephonic support to patients and families
- Reinforcing the Primary Care Provider’s plan of care
- Supporting patients in achieving their health goals
- Addressing barriers to medication adherence
- Coordinating care with other providers
- Documenting patient encounters in the EMR system
Requirements:
- 2+ years of experience supporting patients in a chronic condition management program
- Experience engaging with patients telephonically
- Knowledge of available community resources and programs
- Excellent verbal and written communication skills
- Proficiency in web-based technology and Microsoft Office applications
- Ability to work in a fast-paced environment
- Reliable internet and a home office setup
