Enrollment Specialist

Job Description

Form Health is seeking an Enrollment Specialist to deliver an exceptional patient experience in a telemedicine environment. The Enrollment Specialist will be the welcoming face of Form Health for new patients, guiding them through the initial stages of their journey. This is a fully remote position within the US, with preference for candidates in PST or MST time zones.

Responsibilities:

• Act as the primary point of contact for new patients, providing clear guidance and support.
• Efficiently register and onboard a high volume of new patients.
• Precisely verify patient insurance information.
• Proactively perform outreach to encourage new patients to complete onboarding steps.
• Offer valuable feedback to internal teams to improve operational tools.
• Assist with the training and orienting of new staff members as needed.

Requirements:

• At least two years of experience in a healthcare setting, patient enrollment, or customer service role.
• Strong customer service drive and a genuine passion for helping patients.
• Excellent verbal and written communication skills.
• Exceptional organizational skills and proven prioritization skills.
• Proficient computer skills and ability to adapt to evolving technologies.
