Interwell Health is seeking a Renal Care Coordinator to join their team in Houston, Texas. This role involves working within a local nephrology practice to organize the care of late-stage chronic kidney disease patients. The Renal Care Coordinator will provide support, education, and care coordination services to ensure patients have an optimal start to dialysis.
Responsibilities: - Assesses patient knowledge of late-stage CKD and treatments.
- Educates and informs patients to enable them to make informed decisions.
- Provides support, guidance, and coordination of care for patients seeking conservative or palliative care.
- Acts as a liaison with staff to ensure patients receive comprehensive information on modality advantages and disadvantages.
- Organizes the Nephrology Practice late-stage CKD patient population regarding CKD education.
- Participates in the interpretation of summary clinical data to improve late-stage CKD care processes.
Requirements: - Minimum of 2 years previous experience in clinical renal patient care.
- A combination of renal transplant, dialysis, or CKD patient care required.
- Understanding of diabetes and cardiovascular disease processes preferred.
- Strong organizational and communication skills.
- RN Highly Preferred.
- Bilingual (English/Spanish) a plus!
