Job Description
Forge Health is seeking a Licensed Mental Health Therapist to join their growing team in Doylestown, PA. Forge Health is dedicated to providing high-quality, affordable, and effective care to individuals, families, and communities in need. The therapist will provide mental health services via telehealth and in person, treating addiction and mental health conditions. This role reports to the Clinic Director.
Role Involves:
  • Facilitating outpatient and intensive outpatient programs.
  • Providing individual and family therapy.
  • Performing case management.
  • Assessing clients for co-occurring and mental health needs.
  • Completing initial assessments and determining appropriate level of care.
  • Preparing and maintaining treatment plans.
  • Coordinating with discharge planners.
  • Documenting counseling services.
  • Participating in multidisciplinary team meetings.
  • Conducting individual and group counseling.
  • Communicating with family members and other resources.
  • Providing crisis intervention.
Requirements:
  • Valid, unrestricted license in Pennsylvania as a LCSW or similar (LSW, LAPC, LPC, or LMFT).
  • Master's Degree in Social Work or a similar field.
  • Minimum of two evening shifts per week.
