Intake Representative Supportive Care

Job Description

West Cancer Center is seeking an Intake Representative to join their Supportive Care team. In this role, the Intake Representative will be responsible for processing referrals for home care, hospice, and rehabilitation services. They will verify patient information, assist with coordination of benefits and care needs, and work directly with patients, families, agencies, and internal teams.

Role Involves:
• Receiving and processing referrals for Home Health, DME, Hospice, and Rehab services
• Providing support to patients, caregivers, physicians, and referral sources
• Ensuring data integrity and accurate entry of patient demographics, orders, and insurance details
• Contacting third-party payors for pre-certification and benefit verification
• Maintaining tracking systems and document all interactions in the EMR
• Communicating with home health/hospice agencies across TN, MS, AR, MO, and KY
• Maintaining and updating referral resources and agency contact lists
• Supporting patients with insurance forms, financial assistance, and accessing the patient portal
• Assisting with SDOH and Rehab pre-authorization follow-ups as needed
• Preparing materials for outside agencies and internal coordination

Requirements:
• High school diploma or equivalent required
• Minimum one (1) year of experience in a healthcare environment required
• Experience with insurance verification or accessing coverage information from third-party payors (preferred)
• Strong customer service skills
• Ability to adapt and respond to complex situations
• High attention to detail and ability to manage multiple tasks
• Proficient in Microsoft Word and general office equipment
• Strong written and verbal communication skills
• Ability to work independently and exercise sound judgment
