Obesity Medicine Physician - Remote

Form Health, a virtual obesity medicine clinic, is seeking an Obesity Medicine Physician to provide telehealth care and collaborate with a multidisciplinary team. The physician will deliver evidence-based obesity treatment through telemedicine, working closely with advanced practice professionals and registered dietitians. Form Health is committed to high-quality care, leveraging technology to enhance the patient experience.Role involves:

• Providing longitudinal telehealth obesity medicine and medical weight management care.
• Collaborating with advanced practice professionals and registered dietitians.
• Seeing patients across a 5-day work week.

Requirements:

• MD or DO degree.
• Board Certification in Obesity Medicine (ABOM).
• 2+ years of obesity medicine/medical weight management experience.
• License to practice and prescribe in one or more of the following states: CA, IL, TX, MA, NY, or NJ.
• Excellent interpersonal skills.
• Commitment to high-quality medical care and an evidence-based approach.
• Prior telemedicine experience.
• Experience working collaboratively in a multi-disciplinary fashion.
• Bilingual in Spanish preferred.
