Obesity Medicine Nurse Practitioner

Job Description

Form Health is seeking a dedicated Nurse Practitioner to join their virtual obesity medicine clinic. The Nurse Practitioner will provide longitudinal telehealth care to patients with obesity, collaborating with physicians and registered dietitians to deliver team-based care. This is a full-time, remote position.

• Providing longitudinal telehealth care for patients with obesity.
• Collaborating with physicians and registered dietitians.
• Delivering team-based care.

Requirements:

• Nurse Practitioner degree with experience in Obesity Medicine.
• Licensed to practice and prescribe in at least one of the following states: CA, MA, NY, TX, NJ, and/or IL.
• 2+ years of experience in outpatient or obesity medicine, endocrinology, or bariatrics, or 2+ years of obesity medicine experience in a primary care/internal medicine setting.
• Excellent interpersonal skills.
• Commitment to high-quality medical care and an evidence-based approach.
• Prior telemedicine experience.
• Bilingual in Spanish preferred.
