Job Description
Blackbird Health is seeking a Care Navigator to serve as an experienced care specialist for their patients. The Care Navigator will engage and support patients who have begun their care journey with a Blackbird Provider and support families in creating a shared understanding of the precision health care model that Blackbird provides. They will be a trusted partner to patients and providers, navigating patient care and supporting a positive experience.
Role Involves:
- Fielding inbound calls and answering questions.
- Scheduling and confirming appointments.
- Navigating patient care and ensuring a positive experience.
- Supporting administrative tasks.
- Assisting with medical forms.
- Adhering to confidentiality and data protection standards.
- Attending team meetings and participating in training.
- Independently problem-solving and organizing time.
- Using new technologies.
- Supporting in-person office needs.
Requirements:
- Certified Medical Assistant or degree in Healthcare Administration or related field.
- Minimum 1 year of experience in a medical office or healthcare setting.
- Experience with electronic health records or patient engagement platforms.
- Telephonic outreach and customer service experience.
- Computer skills (Windows, Microsoft, Excel, Google Docs, Google Meet).
- Passion for helping others.
- Efficient work habits and ability to maintain consistent volume.
- Ability to connect with diverse individuals.
- Empathic listening skills.
- Eagerness for ongoing learning and growth.
- Excellent time management and self-organization.
