VEN Centers Outreach Specialist / Phlebotomist (West Valley)

Job Description

Arizona Liver Health (ALH) is seeking a VEN Centers Outreach Specialist/Phlebotomist to combat viral hepatitis in Arizona. The role involves patient outreach and advocacy, aiming to deliver the best care and remove obstacles to treatment for affected patients. This is a remote position requiring 50-75% travel to pop-up sites.

Responsibilities:

• Research and coordinate with local health departments and community groups.
• Conduct outreach to Viral Hepatitis High-Risk communities.
• Provide individualized education and risk reduction plans.
• Complete patient testing for Viral Hepatitis and other STIs.
• Conduct psychosocial assessments.
• Provide support to HCV/STI positive persons.
• Identify and re-engage patients who have fallen out of care.
• Maintain documentation and patient records.
• Arrange and provide group education for Viral Hepatitis/STIs.

Requirements:

• Proficiency in Microsoft applications and EMR systems (eCW).
• Excellent communication skills.
• Ability to work independently in a remote environment.
• Strong interpersonal skills.
• Openness to working with diverse backgrounds.
• Flexibility to adapt to changing schedules.
• Knowledge of public health practices.
• Phlebotomy experience.
• Bachelor's degree in a relevant field.
