Job Description
WellPower is seeking compassionate and dedicated individuals for the role of Clinical Case Manager I/II. This role involves providing comprehensive psychological and behavioral case management services to improve or maintain each mental health consumer’s abilities to function in the community. The Clinical Case Manager assesses, monitors, and follows up with consumer’s ability to meet basic needs in moving toward recovery and is responsible for all clinical documentation of consumer interaction. The position is located in Denver, Colorado.
Responsibilities:
- Helping consumers meet basic needs for food, clothing, shelter, safety, and medical care.
- Assisting with applications for income, medical, housing, or other benefits.
- Arranging transportation for consumers to access needed services.
- Providing comprehensive psychosocial services.
- Guiding consumers in daily living skills (medication, hygiene, budgeting, etc.).
- Helping consumers develop social skills and find employment opportunities.
- Offering support and education to families and community members.
Requirements:
- Bachelor's (CM I) or Master's (CM II) Degree in psychology, social work, or related field.
- One year of experience in mental health or human services preferred.
- Valid Colorado Driver's license and clean driving record.
- Ability to transport individuals served in personal vehicle.
- Ability to obtain Business Use coverage on auto policy (cost reimbursed).
