Case Manager

Job Description

Avenues Recovery Center is seeking a Case Manager for their facility. This role involves advocating for clients, addressing external barriers to treatment, managing client accounts, and facilitating discharge planning. The Case Manager will arrange housing, financial aid, and vocational training as needed.

Responsibilities:

• Advocate for client, addressing and resolving any external barriers to treatment
• Initiate case management for each client upon intake, addressing any pressing needs
• Oversee client accounts, ensuring insurance is active throughout duration of treatment
• Provide ongoing case management throughout the duration of treatment
• Arrange housing, financial aid, legal aid, vocational training etc. as necessary for client post -discharge
• Facilitate comprehensive discharge planning (continuing care, housing, sober support etc.)

Requirements:

• High School diploma or GED preferred
• Min. 1-year experience in treatment industry preferred
• If in recovery, a minimum one-year period of sustained sobriety is required.
• Excellent communication, multitasking, and time management skills
• Strong problem-solving and critical thinking skills
• Warm, empathetic, respectable personality
