Herself Health is seeking a Primary Care Nurse Practitioner or Physician Assistant to join their team in Eagan, MN. This full-time role involves evaluating and managing patients' acute and chronic conditions, including medication management, mental health support, and patient education. The Nurse Practitioner/Physician Assistant collaborates with the care team to deliver high-quality, patient-centered care to women 65 and older.Role Responsibilities:

• Evaluate patients’ acute and chronic conditions.
• Manage medications and address mental health needs.
• Educate patients and partner with the care team.

Required Qualifications:

• Licensed Nurse Practitioner or Physician Assistant.
• 3-5 years of experience managing complex diseases across time.
• Understanding of value-based care.
• Experience educating patients on health maintenance.
• Experience with geriatric medicine.
• Strong problem-solving skills.
