Herself Health is seeking a Primary Care Nurse Practitioner/Physician Assistant to join their team in Crystal, MN. This role is crucial in providing comprehensive care to women aged 65 and older at Crystal Clinic. The ideal candidate will have experience in managing chronic conditions specific to this demographic and a passion for improving women's healthcare.About the Role:

• Evaluate patients’ acute and chronic conditions.
• Manage medications and mental health needs.
• Educate patients in a thorough and empathetic manner.
• Partner with the care team to provide excellent patient care.

Requirements:

• Licensed Nurse Practitioner (FNP-C or AGNP) or Physician Assistant (PA-C).
• 3-5 years of experience managing complex diseases.
• Understanding of practicing in a VBC model.
• Experience devising individualized care plans.
• Experience with geriatric medicine.
• Experience with poly-pharmacy management.
• Strong problem-solving skills.
• Up-to-date knowledge of medical advancements.
• COVID-19 vaccination.
