Herself Health is seeking a part-time Primary Care Physician to join their team in Minneapolis/St. Paul, MN. Herself Health is building a new model of primary care for women 65+.
The Primary Care Physician will be responsible for evaluating patients' conditions, managing medications, mental health, and socio-emotional needs, and providing patient education.
Role Involves:
- Evaluating patients’ acute and chronic conditions.
- Managing medications, mental health, and socio-emotional needs.
- Providing patient education.
- Collaborating with clinical teammates.
- Assisting with development, implementation and achievement of key organizational objectives and metrics.
Requirements:
- Licensed MD/DO with board certification in Family Medicine or Internal Medicine.
- 3-5 years of experience managing complex diseases.
- Understanding of value-based care.
- Experience managing overall patient care and devising individualized care plans.
- Experience educating and counseling patients on health maintenance and disease prevention.
- Experience with geriatric medicine, particularly women.
