Primary Care Physician (Per Diem)

Herself Health is seeking a Per Diem Primary Care Physician to join their team in Minneapolis/St. Paul, MN. Herself Health is dedicated to providing specialized primary care for women 65+ focusing on post-menopausal care, bone density, weight management, and mental, social, and emotional well-being.

The Primary Care Physician will be responsible for evaluating patients' acute and chronic conditions, managing medications and mental health, and providing patient education. The role involves collaborating with clinical teammates to deliver comprehensive care and contributing to organizational objectives within a value-based care model.

Responsibilities:

• Evaluate patients' acute and chronic conditions.
• Manage medications and mental health needs.
• Provide patient education.
• Collaborate with clinical teammates.
• Assist with development, implementation and achievement of key organizational objectives and metrics

Requirements:

• Licensed MD/DO with board certification in Family Medicine (ABFM) or Internal Medicine (ABIM).
• 3-5 years of experience managing complex disease.
• Understanding or willingness to learn how to practice in a VBC model.
• Experience managing overall patient care and devising individualized care plans.
• Experience educating and counseling patients on health maintenance and disease prevention.
• Experience with geriatric medicine, particularly women.
• Experience with poly-pharmacy management.
• Experience with physical and gynecological/well-woman examinations in the elderly population.
• Strong problem-solving and critical thinking skills.
