Author Health is seeking a Facilitated Telehealth Coordinator to serve as an in-person liaison between patients and the virtual care team in Jacksonville, Florida. The coordinator will improve health and well-being through care coordination and technology facilitation.

- Traveling to patient homes or primary care offices to assist with telehealth visits.
- Facilitating communication between providers and patients.
- Assessing environments for safety concerns and communicating patient needs to the care team.
- Coordinating telehealth services and providing education and support to patients and caregivers.
- Maintaining records, collaborating with IT, and conducting training sessions.
- Troubleshooting and resolving technology issues.
Requirements:
- Comfort working with seniors and those with behavioral health conditions.
- Experience in a healthcare setting and knowledge of telehealth technology.
- Excellent organizational, communication, and problem-solving skills.
- Ability to work independently and travel up to 100 miles round trip per day.
- Driver’s license and car.
- Preferred: Bilingual (Spanish-English) and experience with mental health conditions.
