Outreach Liaison - Jacksonville, Florida at Author Health

Author Health is seeking an Outreach Liaison in Jacksonville, Florida, to connect patients with mental health and social services. The candidate will build trust with patients with SMI, SUD, and dementia and help them schedule appointments. They will also expand Author Health’s network of referral partners.

What the role involves:

• Conducting in-person outreach to educate potential patients and caregivers.
• Organizing workload to meet outreach and enrollment goals.
• Tracking outreach and documenting member interactions in the CRM.
• Collaborating with internal outreach and care teams.
• Aiding in capturing data and providing feedback on outreach strategies.
• Participating in virtual meetings.

Requirements:

• 3+ years of customer service/sales experience
• 3+ years of healthcare outreach, community engagement, or related
• Multi-lingual (Spanish and/or Haitian Creole) preferred
• Strong ability to build connections quickly.
• Experience in healthcare, social services, or mental health services.
• Proficiency in critical thinking, time management, and multitasking.
• Able to work independently and meet monthly outreach goals.
• Enthusiasm for working in a startup environment.
• Access to a car and ability to drive within a 100-mile radius.
• Strong tech proficiency.
• Basic proficiency in Google Suite and video conferencing.
• Adherence to HIPAA regulations and confidentiality.
