Nurse Practitioner (Evening & Weekends)

Dialogue is seeking a Nurse Practitioner to join their team. In this role, the Nurse Practitioner will play a crucial part in accompanying patients through their healthcare journey by providing front-line medical support and symptom evaluation.

The Nurse Practitioner will collaborate with a multidisciplinary team of medical professionals, contributing to a culture of continuous learning and teamwork within Dialogue.

What this role involves:

• Conducting remote assessments via the Dialogue Care Platform.
• Delivering high-quality patient care by offering timely medical advice.
• Creating personalized care plans tailored to patient needs.
• Collaborating with an interdisciplinary team.
• Educating and supporting patients on preventative care.
• Referring patients to specialists or allied healthcare professionals and community services.
• Maintaining detailed records of patient interactions, treatment plans, and follow-ups.
• Contributing to internal projects or initiatives.

Requirements:

• Must be comfortable consulting in French or English.
• Availability to work evenings and weekends for a minimum of 8 hours per week.
• Master's degree in Nursing, advanced practice option and a complementary diploma in primary care.
• Hold a valid and active medical practice license in Quebec, Manitoba, or Saskatchewan.
